Getting Ready for Your Visit
In order for us to provide the best care for you, it is very important that you bring the following items to your visit:
- Photo ID
- A list of your current medications
- Any and all x-rays films
- Insurance information and card
- Information about what treatments and medications have been tried in the past
- Medical records from prior visits that you think may be helpful
- New Patients: download a patient registration below
Secure Online Patient Forms
Patient Portal
OSS Health has implemented a Patient Portal to enable you to access your medical information in a secure manner via the internet. Following are just some of the functions that are available via the Patient Portal:
- Update demographic and insurance information
- Update current list of medications
- Send non-urgent messages to the physicians and/or staff
- View and request upcoming appointments
- Request a medication refill
- Add or remove allergies
- Add or update past medical, family or social history
To activate your portal, please request an activation code via e-mail to it@ossortho.com. After you receive the activation code, go to www.ossortho.com, click on the call-out on the front page that says Access Our Patient Portal. Click on New User and then click on Activate Patient Portal Account. The following information will be needed:
- Name
- Date of birth
- E-mail address
- Create a login and password
- Enter activation code provided by OSS
- Update security information
- Select an ID and phrase
